Radiation therapy after mastectomy can cause wound healing problems in reconstructed breasts. However, it is a wise decision to delay the breast reconstruction, especially if a patient has undergone inflammatory breast cancer surgery until after radiation therapy is completed. In addition, due to advancements in techniques and surgical improvements, radiation therapy is no longer a restriction for breast reconstruction surgery, approving the possibility of immediate reconstruction with implants. Moreover, autologous tissue breast reconstruction is usually reserved for women who have had radiation therapy to improve the damage caused by radiation therapy to the breast and chest wall tissue. Secondly, immediate reconstruction is not a preferable option for women with inflammatory breast cancer, which usually requires more extensive skin removal, recommending the pause of breast reconstruction surgery until after the completion of adjuvant therapy. However, delayed reconstruction is a preferable option for women with inflammatory breast cancer.

    I. Implant-based breast reconstruction:

    Implants are prostheses placed underneath the skin or chest muscle following mastectomy. Implants are of two types, i.e., silicone shell filled and saline (sterile saltwater) or silicone gel. We prefer silicone implants at Dr. Karishma Aesthetics because they give a more natural effect and are softer than saline implants to the touch.

    Key points for the reconstruction of breasts with implants must be thoroughly informed while deciding on an option:

    • There must be enough skin and muscle after mastectomy to cover the implant.
    • It is a shorter procedure than autologous tissue reconstruction, and less blood loss is reported.
    • Frequent follow-up visits may be required to inflate the expander and insert the implant.
    • The recovery period is shorter.
    • It may not be a preferable option for women who have had radiation therapy.
    • Implants will not last a lifetime and may need revision surgery in case of wear and tear issues or replacement after 10-12 years, as maintenance with periodic follow-up visits is required post-surgery.
    • In the case of immediate reconstruction, when breast reconstruction is done following mastectomy, placing a tissue expander under the skin or chest muscle is not required, as an implant can be placed in the breast during the same surgery.
    • Women with silicone implants are required to undergo periodic MRI screenings to avoid the risks of possible “silent” rupture of the implants, as recommended by the FDA.

     

    II. Autologous tissue/ flap Reconstruction:

    Autologous tissue reconstruction helps replace damaged chest wall tissue with healthier one from elsewhere in the body where extra fat is localized, and easier to obtain flaps such as the abdomen, thighs, back, buttocks, or love handles.

    Women who previously had abdominal surgery may not be the right fit for an abdominally-based flap reconstruction.

    Key points for the reconstruction of breasts with Autologous tissue in the view of surgery and recovery:

    • Autologous tissue breast reconstruction involves longer surgical hours than implants, as the first step is about harvesting fat and obtaining skin, blood vessels, and muscle from donor areas. Then, it is used to reconstruct the breasts. 
    • When tissue/ flap is obtained from the thighs, back, or buttocks, it is called a free flap often required when a woman has had a previous major abdominal surgery and does not have enough tissue to reconstruct the breasts. Moreover, a free flap often requires an implant to provide the desired breasts’ volume.
    • Recovery from the procedure and healing period is longer than implants.
    • One should be physically fit to undergo autologous tissue reconstruction, as obesity, diabetes, and smoking may increase the rate of complications.
    • Pain and weakness at the donor site from where the flap is harvested are usually common and can be managed effectively with prescription medications and post-op measures.
    • Autologous tissue transfer gives a more natural breast shape than implants and may feel softer and more natural to the touch.
    • Autologous fat tissue reconstruction can replace damaged tissue caused by radiation therapy.

    Post-mastectomy Recovery Guidelines:

    Mastectomy or breast removal surgery is performed to treat breast cancer in women, and the procedure lasts one hour or more depending on the extent of the procedure, the type of reconstruction technique used, the anatomical status of the patient, and the problem status. Usually, the recovery from mastectomy takes around 4-6 weeks for the wound to get completely healed and associated discomforts or side effects to be settled. However, strenuous activities or lifting over 10 pounds should be paused for four weeks, and limited arm movements until the surgical drains are removed is instructed. Moreover, recovery from a mastectomy without reconstruction typically takes about two weeks, while a mastectomy with reconstruction often takes 4 to 6 weeks.

    Post-mastectomy instructions:

    • Allow yourself plenty of rest in the first few weeks following surgery to heal smoothly.
    • Take prescription pain medications as needed.
    • Sleeping on your back is prescribed until you achieve a fuller recovery, i.e., 4-6 weeks.
    • Getting enough sleep will help you heal faster.

The issue surrounding the optional nature of Input Service Distributor (ISD) and cross-charging, previously resolved by a government circular allowing companies to choose whether to adopt ISD, has been reignited due to a new government proposal mandating the use of ISD. This change means companies will now face an increased compliance burden, as they will be required to register for ISD and determine situations where cross-charge invoices need to be issued between branches that share the same PAN but have different GSTN numbers.”

    Budget 2025 on HRA, Income Tax, Tax Slabs and ITR

    As predicted, Union Finance Minister Nirmala Sitharaman has suggested keeping the current tax rates for import tariffs, direct taxes, and indirect taxes in the year of the 2024 General Elections. She stated in her address on Budget 2024, “In keeping with convention, I do not propose to make any changes relating to taxation and propose to retain the same tax rates for direct and indirect taxes, including import duties.”

    “However, certain tax benefits to startups and investments made by sovereign wealth or pension funds, as also tax exemption on certain income of some IFSC units, are expiring on March 31, 2024; to provide continuity, I propose to extend the date to March 31, 2025,” according to her.

    • Always wear prescribed moisturizers containing a lower vitamin A dose, zero fragrance, and loads of soothers.
    • Always use retinol in conjugation with sunscreen, which means apply retinol at night and wear sunscreen every day regardless of whether you are stepping out.

    Note: Retinols work on the top layer of the skin, promote skin cell turnover, brighten skin tone, reduce acne, and boost collagen production.

    • Since retinol is a highly active ingredient, skipping your retinol product the day before you exfoliate is recommended. So, suppose you are scheduled for certain in-office treatments, like microneedling, microdermabrasion, chemical peels, or laser. In that case, you are instructed to pause the application of retinol a few days before-and-after treatments to avoid unwanted skin irritation or harsh effects.
    • If your skin is too sensitive and you have concerns like Rosaceae or eczema, it is not recommended to use retinol; instead, you can go for Bakuchiol or wild indigo, which works beautifully without any irritation or sun sensitivity.
    • If someone has acne concerns from the very beginning, they can use retinol in their teens or twenties. But, mid-twenty is the best time to start retinol for those with concerns like early signs of aging, such as sun spots, discoloration, age spots, and acne.

    In the context of slowing down the aging process and improving the tone, texture, and collagen and elastin production, retinoids are 10x more bioavailable than retinol. Retinoids are pricey prescription medications only recommended after evaluating skin concerns and type. Retinoid is a “catch-all” term for all vitamin A-based products on the skin.     Dr. Karishma Kagodu: Following the completion of my MBBS, a pivotal decision confronted me in the trajectory of my educational journey – the choice between pursuing surgical or non-surgical fields. During the period when I graduated from MBBS, there was a prevalent trend, particularly among female peers, to opt for non-surgical medical professions.

    Despite the prevailing inclination, my passion for surgery, specifically plastic surgery, prevailed. In defiance of family expectations, which urged me towards specialties such as dermatology or pediatrics, my unwavering interest in surgery led me to pursue a specialization in general surgery.

    Motivated by a longstanding fascination with beauty and aesthetics, I envisioned myself as a fitting candidate for plastic surgery. During my tenure in general surgery training, I had the privilege of working under a mentor whose discerning eye recognized my proficiency in delicate procedures, particularly well-suited for plastic surgery.

    Encouraged by his insights and the opportunities presented, I embarked on a few months of collaborative work, affirming my mentor’s belief that my talents were best aligned with plastic surgical specialties.     How has your background in general surgery influenced your perspective as a plastic surgeon?

    Dr. Karishma Kagodu: Upon entering the field of general surgery, I observed that in India, it predominantly adhered to a male-dominated paradigm. This gender disparity persisted even in 2006, when I obtained my MBBS, and continues to characterize the profession.

    Throughout my tenure in general surgery training, students and residents underwent diverse super-specialty rotations, including exposure to neurosurgery, urology, cardiac surgery, liver surgery, pediatric surgery, and notably, plastic surgery.

    During these rotations, I had the opportunity to collaborate with various specialists, and it became evident to me that plastic surgery resonated most with my professional aspirations. Despite initial considerations for neurosurgery, the higher incidence of mortality, morbidity, and fatalities in that field led me to reassess my preferences.

    In contrast, plastic surgery presented a distinct environment characterized by meticulous and refined procedures, intricate tailoring, and a profound emphasis on artistic expression. The convergence of artistry and aesthetics in plastic surgery significantly captivated my interest.

    As I neared the conclusion of my residency in general surgery, I unequivocally committed to pursuing plastic surgery, recognizing its demand for finesse and artistic proficiency.
